Azure Cosmos DB: Support for index policies and .NET SDK 1.3.0

This Azure Cosmos DB update includes support for online index transformations. The Azure Cosmos DB REST API supports PUT on collection resources, and the .NET SDK version 1.3.0 supports ReplaceDocumentCollectionAsync for updating indexing policies.
